Third line - 1804 = date of assessment.
Second side:-
First line- ЛД = LD = l'nyanoy dosmotr = Flax Inspection.
Second & Third line - A?.CE = A?.SYe : H?HЬ = N?N name of quality control officer.
Fourth line - H*5 = N*5 + number of work station used by QA officer.
